Our pharmacists visit clients regularly in their homes to monitor and provide education on one’s medical conditions and medications. Visits may be as regular as once weekly, bi-weekly or monthly, depending on the client’s needs. The visits are frequent, so we can develop rapport with the client, enabling us to be more effective in helping one achieve their healthcare goals.
During a visit, we may identify a drug-related problem, and we will collaborate with the prescriber & other pertinent healthcare providers to resolve the issue.

Drug Information Support
Our pharmacy has access to a depth of literature, including UpToDate, RxFiles, Therapeutic Choices, Natural Medicines Comprehensive Database, Lexi-Comp, and others.
We pride ourselves in having our pharmacists stay up-to-date and can help answer any drug-related questions you have.






In-home Patient Monitoring
Our pharmacists regularly see clients in their homes – weekly, bi-weekly or monthly, depending on individual needs. A home visit enables the pharmacist to monitor your patient for compliance, medication overuse, medication misuse, side effects, or therapeutic effectiveness of a medication.
The pharmacist often solves Medication-related problems through repeated, frequent visits that involve educating, counselling and introducing medication-reminder packaging (e.g. bubble packs or compliance packs).
Some problems may require your attention and consent. We may make recommendations if we see a problem with the therapeutic effect of one’s medication. Our pharmacists have the resources to stay up to date, so any advice comes from the best available, most current evidence.






In-home Blood Pressure Checks
You may want your patient to get “in-home” blood pressure readings to compare it to what you take in the clinic.
For some patients, it can be challenging to access a machine outside of your office. This may be because they cannot afford their medication, commute to a pharmacy, or have difficulty taking their blood pressure correctly.
We can visit your patient in their home to take regular blood pressure readings and forward you the results.






In-home Injection Services
We can administer injections intramuscularly and subcutaneously in your patient’s home regularly. This includes parenteral medications and vaccines. Learn More!






Medication Reviews and Recommendations on Prescribing & Deprescribing
Our pharmacists regularly perform medication reviews when they visit your patient in their home. Sometimes an adverse effect, an untreated issue, an interaction, poor efficacy, or a medication that may no longer be indicated is identified.
We will bring these issues to your attention and make recommendations for the next best course of action.
We can help to deprescribe medications you feel are now harmful or unnecessary. We understand there is no “one size fits all” approach, and patients vary widely in how fast they can taper off the medication. We work closely with your patient to provide encouragement, counsel on expected withdrawal effects, and adjust tapering plans as required.
